Terms and Conditions
Elevate Dental is pleased to introduce our Patient Referral Program, which rewards our valued patients for referring new clients to our clinic. By participating in this program, patients can earn points based on the spending of the individuals they refer, which can be redeemed for discounts on dental services, the purchase of clinic products, or transferred to another patient.
Program Rules
- Eligibility
- All existing patients of Elevate Dental who have completed at least one treatment or consultation are eligible to participate in the Patient Referral Program.
- Employees of Elevate Dental and their immediate family members are not eligible to participate.
- The program is designed to reward patients who refer new clients to Elevate Dental. Self-referrals or referrals of existing patients are not eligible for rewards.
- Referral Validity
- For a referral to be considered valid, the referred individual must attend and complete their first scheduled appointment at any Elevate Dental clinic.
- The referred patient must indicate the referrer’s name during the appointment booking process, either online, by phone, or in person. Referrals mentioned post-appointment will NOT be eligible for points.
- There is no limit to the number of referrals a single patient can make, and each successful referral will earn points as per the Referral Points System.
- Points Expiration
- Points awarded under the Patient Referral Program will remain valid for a period of 12 months from the date of issue.
- If the points are not redeemed within this 12-month period, they will expire and will not be reinstated or refunded in any form.
- Patients are encouraged to monitor their point balances and expiration dates, which can be checked by contacting the clinic directly.
- Transferability
- Points earned through the referral program can be transferred to another Elevate Dental patient. The cash equivalent value will be based on the conversion rate set by Elevate Dental.
- To request a transfer, the patient must submit a formal request either in writing or verbally in person at any Elevate Dental clinic. The request should include the full name and contact details of the recipient patient.
- Transfers will be processed within 5-7 business days, subject to approval and verification by Elevate Dental. The recipient will be notified once the transfer is complete.
Once transferred, the points cannot be re-transferred or converted back into points by the original holder.
- Exclusions
- Points cannot be earned on referrals where the referred patient is a current or past patient of Elevate Dental.
- The referral program does not apply to referrals made before the program’s official launch date.
- Points cannot be redeemed on treatments or products that are already subject to other discounts, promotions, or special offers unless otherwise stated.
Referral Points System
Points will be awarded based on the total amount spent by the referred patient. The following scale will apply:
- For spending between ₱1,000 to ₱9,999 – 50 Points
- For spending between ₱10,000 to ₱29,999 – 250 Points
- For spending between ₱30,000 to ₱74,999 – 450 Points
- For spending between ₱75,000 to ₱149,999 – 750 Points
- For spending ₱150,000 and above – 1,050 Points
Points are calculated based on the net amount paid by the referred patient after any discounts, insurance claims, or other adjustments.
Points will be credited to the referrer’s account after the referred patient completes their appointment and payment is received in full.
Referral Rewards
- Discounts on Dental Treatments
- Points can be used to obtain discounts on the referrer’s next dental treatment. The discount value will be determined based on the total points accumulated.
- The discount can be applied to any dental service offered by Elevate Dental, subject to availability and at the clinic’s discretion
- Purchase of Dental Products
- Points can be redeemed to purchase available dental products at any Elevate Dental clinic, including but not limited to oral care products, whitening kits, and dental accessories.
- Product availability is subject to stock levels, and certain items may be excluded from the program.
- Transfer to Another Patient
- Points may be converted to a cash equivalent value and transferred to another Elevate Dental patient, as outlined in the Transferability section.
- The cash equivalent can be used by the recipient for any eligible dental service or product purchase at Elevate Dental.
General Terms
- Program Modifications and Termination
- Elevate Dental reserves the right to modify, suspend, or terminate the Patient Referral Program at any time, with or without prior notice. Changes may include but are not limited to point values, reward structures, and eligibility criteria.
- Any changes to the program will be communicated through our website and in-clinic notices. Patients are encouraged to review these terms regularly.
- Fraudulent Activity
- Elevate Dental strictly prohibits any form of fraudulent activity, including but not limited to the creation of fake referrals or the manipulation of referral information.
- Any patient found to be engaging in fraudulent activity will have their points forfeited, and they may be disqualified from the program. Legal action may also be pursued where applicable.
- Data Privacy
- Personal information collected as part of the Patient Referral Program will be used solely for the purpose of administering the program and will be handled in accordance with Elevate Dental’s privacy policy.
- Limitation of Liability
- Elevate Dental is not responsible for any technical errors or malfunctions that may affect the operation of the Patient Referral Program, including issues with point tracking or redemption.
- Acceptance of Terms
- Participation in the Patient Referral Program constitutes the patient’s acceptance of these Terms and Conditions. If a patient does not agree to these terms, they should not participate in the program.
