Introduction

Welcome to Elevate Dental’s Patient Referral Program. We appreciate your loyalty and trust in our dental services. This program is designed to reward our valued patients for referring new clients to our clinic. Please read these Terms and Conditions carefully to understand how the program works and the rules governing it.

Eligibility

Participation in the Patient Referral Program is exclusively open to individuals who are current or former patients of Elevate Dental.

You must be at least 18 years old to participate.

Referral rewards will only apply to the following advanced dental services:

• Invisalign Orthodontic Treatment
• Oral Surgery Procedures
• Veneers and Crowns Application
• Braces and Orthodontic Care

Referral Rewards

Referrers will receive a credit allowance worth P5,000 for every successful referral that results in a net transaction value of P100,000 or more.

Successful referrals are defined as individuals who have not previously received dental services at Elevate Dental and who have completed eligible dental treatments as described above.

The values will be audited by the company’s Finance Officer to ensure accuracy and fairness.

Multiple referrals are encouraged and will result in multiple rewards for the referrer.

Note: Referral rewards will be issued for new patients only. Existing patients referred to the program are not eligible for rewards.

Referral Process

To refer a new patient, one must accomplish the Patient Referral Form in the company website’s landing page (see form above). Contact details and personal information must be provided for both referrer and patient referral. 

The patient will be contacted by Elevate Dental’s Front Desk Officer to validate all the details and schedule the appointments / procedures.

Payout Timeline

Issuance of the credit allowance for successful referrals will be processed within 30 days after the referred patient has made full payment for the dental service(s) rendered by Elevate Dental.

Referral credits are valid for 12 months from the date of issue. If not used within the validity period, the credit allowance will expire and become null and void.

Elevate Dental reserves the right to extend the credit timeline under exceptional circumstances.

Redemption of Referral Rewards

The referrer’s credit allowance can be redeemed from any participating Elevate Dental clinic.

The credit allowance can only be redeemed for dental services and cannot be exchanged for cash or any other form of compensation.

Credit allowances are non-transferable and can only be used by the referrer.

The referrer is required to formally notify Elevate Dental’s Front Desk Officer of their intent to utilize their credit allowance with a lead time of 3-5 days prior to their scheduled dental appointment.

Program Changes and Termination

Elevate Dental reserves the right to modify or terminate the Patient Referral Program at any time, with or without notice.

In the event of program termination, referral rewards will be processed for referrals made up to the termination date.

Compliance with Laws

All participants in the Patient Referral program agree to comply with all applicable laws and regulations.

Elevate Dental may withhold or cancel referral rewards if any participant is found to be engaging in fraudulent or illegal activities.

By participating in Elevate Dental’s Patient Referral Program, you agree to these terms and conditions. Elevate Dental reserves the right to make changes to these terms as necessary, and any updates will be posted on our website.
